2. By calling toll free: 800-244-9373 and requesting a Personal Financial Profile Booklet.
3. By providing data online in the secure portion of this site (see special note below).

Special Note For Submitting Personal Financial Data Online:
Any information that you include in the online questionnaire is not available to us or anyone else until you press the submit button. Then it is only available to Dorion-Gray. Pursuant to all applicable privacy laws and our own strict privacy policy, all information is kept strictly confidential and is never made available to any other person, company, or organization. A copy of our privacy policy is available on this web-site. You need to enter an email address and a password of your choosing, so that if you do not have the time to complete, or need to obtain additional information to complete, you will be able to access your file at a later date. No one can access your file, even Dorion-Gray, until you submit it. If you do not finalize and submit your file within 30 days, it will automatically erase. Upon receipt of your information, we will contact you to set-up your free, no-obligation personal evaluation at one of our two local area offices. We look forward to meeting with you.
